Request a Change of Business Name, Address, and/or Account Status

Use this form to notify the Department of Revenue of:

a change of address.
a change in your tax account status.
a change in business name (no change in legal entity or business ownership).

Use a Florida Business Tax Application (Submit online or download form DR-1) for:

a change in legal entity.
a change of ownership.
a change of business location address from one Florida county to another.

Instructions:

Choose the type of change you are requesting.
Complete Section 1 to identify your account(s).
Check the box next to the tax(es) in Section 2 for which this change request applies.
For business name changes, complete Section 3.
For address changes, complete Section 4.
For account status changes, complete Section 5.
Provide contact information in Section 6.
Review your request for accuracy, scroll to the bottom of the page and click submit.

5. Change your account status. Select the appropriate action and provide the effective date of the change. Choose only one*:

Inactivate-change from active to inactive status. You are temporarily suspending business activities; you have no employees at this time; you plan to resume activities at a later date

Reactivate-change from inactive back to active status. You are resuming business activities or you are paying wages again.

Cancel-change from active or inactive to cancelled status. You have no plans for future business activity or you no longer have employees. Cancellations cannot be reversed.

In Business, but Not Required to File (Corporate Income Tax ONLY) -change from active status for corporate income tax to in business, but not required to file. The corporation is still in business, but it is not required to file a Florida corporate income/franchise tax return. Enter the last day of the final taxable year you were required to file in Florida for the effective date below.

IMPORTANT NOTE: If you inactivate or cancel your account, you must file a final return and pay all applicable taxes due within 15 days of your inactivation or closing date. Your final return must cover the period from your most recent return filing to the inactivation or closing date.
